By signing this document, you are waiving certain legal rights, including the right to sue. Please read carefully.

1. Acknowledgment of Risks

The undersigned participant or parent/guardian ("Participant/Parent/Guardian") acknowledges that participating in Virtual Reality (VR) activities involves inherent risks, hazards, and dangers, including but not limited to:

• Physical Risks: Eye strain, motion sickness, disorientation, nausea, loss of balance, dizziness, and involuntary movements.
• Collision Risks: The risk of falling, striking, or colliding with walls, furniture, equipment, or other participants and objects within the physical play area, which may lead to serious injury, concussion, or property damage.
• Equipment Risks: Possible malfunction or misuse of the VR equipment (headsets, controllers, sensors) leading to injury.

2. Health and Physical Condition

The Participant/Parent/Guardian affirms that the participant:

• A. Are not currently under the influence of alcohol, drugs, or any medication that could impair their judgment or physical ability.
• B. Have been advised that VR use is not recommended for individuals with certain medical conditions, including but not limited to: serious heart conditions, epilepsy, seizures, balance disorders, pre-existing neck or back injuries, or pregnant individuals.
• C. Will immediately cease VR use and notify an attendant if they experience any discomfort, pain, dizziness, nausea, or disorientation.
• D. Have sufficiently stable physical and mental health to safely engage in the VR activity.

3. Rules and Safety Compliance

The Participant agrees to strictly abide by all rules, regulations, and verbal instructions provided by the staff of Play VR ("The Operator") regarding the safe and appropriate use of the VR equipment and play area. This includes, but is not limited to:

• Staying within the designated play boundaries (Guardian System).
• Wearing the head straps tightly and properly.
• Moving slowly and deliberately while using the equipment.
• Immediately following instructions to stop if requested by an attendant.

4. Release and Waiver of Liability

In consideration of being permitted to participate in the VR experience, the Participant/Parent/Guardian, for themselves and their heirs, personal representatives, and assigns, voluntarily agree to:

• A. Assume full responsibility for any and all risks, injuries (including death), and damages, known or unknown, which may arise during or result from their participation in the VR activities, even if caused by the negligence or fault of The Operator.
• B. Release, waive, discharge, and covenant not to sue The Operator, its owners, directors, officers, employees, agents, and representatives (collectively, the "Releasees") from any and all claims, demands, costs, actions, and causes of action arising from the Participant’s participation in the VR experience.

5. Indemnification

The Participant agrees to indemnify and hold harmless the Releasees from any and all liabilities, claims, demands, judgments, losses, costs, or expenses (including legal fees) that arise out of or are in any way connected with the Participant’s misuse of the equipment or reckless disregard of safety instructions.
